
Dementia Care
Caring for a loved one with dementia can be both deeply meaningful and incredibly challenging. Our Dementia Care services are designed to support individuals living with cognitive conditions such as Alzheimer’s disease and other forms of dementia, while also supporting the families who care for them. We provide compassionate, personalized care that focuses on safety, comfort, and maintaining the highest possible quality of life. Our services include specialized in-home support and respite care, giving family members and caregivers the opportunity to rest, recharge, and tend to their own needs—knowing their loved one is in capable and caring hands. These are non-medical services and are usually paid by VA, Long-Term Care Insurance or Privately Funded.
The GUIDE Model Program

The CMS GUIDE Model is a Medicare program from the Centers for Medicare & Medicaid Services designed to improve care for people living with dementia. It provides funding to healthcare organizations to coordinate comprehensive dementia care, support caregivers, and help patients remain safely at home while reducing unnecessary hospitalizations and emergency visits.
Who Qualifies for GUIDE:

Traditional Medicare Part A and Part B (MA Plans not included)​

Not enrolled in Hospice

Not residing in long-term care facilities

Confirmed or suspected dementia diagnosis

Unpaid caregiver available to aide in assessment and care plan

Must complete comprehensive assessment
