The Centers for Medicare & Medicaid Services recently announced the launch of the Guiding an Improved Dementia Experience – or GUIDE Model – a comprehensive new payment and care delivery initiative aimed at transforming dementia care for Medicare beneficiaries and their caregivers.

As the aging population and prevalence of Alzheimer’s disease and related dementias continue to grow, the Centers for Medicare & Medicaid Services is exploring innovative strategies to enhance quality, coordination, and support while reducing unnecessary costs.
Madison Family Companies is directly involved in this model and recently served on a statewide panel in conjunction with the Texas Association for Home Care and Hospice (TAHC&H).
The primary goal of the GUIDE Model is to establish a standardized, integrated approach to dementia care that combines medical management, caregiver support, and community-based services.
Recognizing that family members provide most of the dementia care and often experience significant stress, the model includes structured caregiver education, ongoing support, and access to respite services.
